No, nobody has died at a park in North America because a roller coaster has malfunctioned. A few people have died because they were dumb and got out of the restraints or didn't secure them (on purpose). At Canada's Wonderland the worst accident was on Jimmy Neutron's Brainwasher (or what it used to be called) where one of the arms flew off, the car fell five feet, two children were hurt and an adult had back problems for a month or so. All the other accidents that have occurred were minor and caused by stupidity or recklessness (broken shoulder, one eye swollen shut, and a fractured nose). So just don't be stupid and you'll be fine.
